Diver Medical Questionnaire – Dive RAID UK & Malta

RAID Professionals are now required to complete the online medical questionnaire every 12 months and upload any accompanying documents onto their profile as required.

Once the medical questionnaire has been updated and any relevant files uploaded the RAID professional will be able to see the expiry of their medical by viewing their medical log (located in the “My Profile” drop down menu).

RAID Professionals should also upload their valid HSE medical certificate to accompany their medical questionnaire.

RAID student divers are required to complete the online medical questionnaire every 6 months, with a similar process of viewing and updating their medical record in the “My Profile” drop down menu. Any required medical documents can be uploaded to a students profile by either the student themselves or the dive centre they are affiliated to.
